CDCES & Registered Dietitian
The certified diabetes care and education specialist (CDCES) delivers comprehensive and seamless services that bridge the gap and integrate clinical and self-management aspects of diabetes care. The CDCES is an integral part of the care team and provides collaborative, comprehensive and person-centered care, and education conducive to behavior change and improved quality of life across the lifespan. The CDCES promotes self-management to achieve individualized behavioral and treatment goals that reduce risks and achieve better health outcomes. The dietitian at BCH is an integral part of the team, where we work together to optimize the care of the individual living with diabetes, from multiple different perspectives, to provide the best resources possible to help people with type 1 diabetes.
Our program currently treats adults with type 1 diabetes in Florida. We have a multidisciplinary team of providers, including an LCSW, Endocrinologist and CDCES. Our services are offered at no charge, and we do not bill. Removing ourselves from the fee for service model allows for flexibility to do what is right for the patient, including longer appointment times and a spectrum of services.
This is a full time, remote, home-office based role eligible for a full benefits package including paid holidays, PTO and competitive pay in a mission-based work environment. Schedule is 11:00am-8:00pm EST / 8:00am- 5:00pm PST Monday- Friday. A private, quiet, home office space is required for patient appointments.
Core Responsibilities:
- Coordinates with BCH Care Team to effectively integrate medical nutrition therapy into the overall management of diabetes.
- Creates individualized medical nutrition therapy plans for patients, throughout the lifespan.
- Is able to create specialized diet plans, for instance for those with celiac disease, chronic kidney disease, and/or gastroparesis.
- Teaches carbohydrate counting and modifies carbohydrate and correction ratios based on the patient’s ability to adjust their insulin doses and integrate changes with data obtained from CGM, connected insulin pens, pumps and hybrid closed loop technologies.
- Works collaboratively with the BCH team to design an appropriate nutritional approach, taking into account the patient’s lifestyle, cultural needs, preferences and challenges in achieving their goals, particularly regarding issues of food insecurity.
- Partners with patients to create nutritional and weight goals and track dietary intake.
- Understands dietary and insulin adjustments necessary for safe and successful exercise in individuals with T1D.
- Documents all individual contacts/visits in the Welkin (EHR) according to the guidelines in a timely manner.
- Provides culturally competent education and counseling tailored to the needs of diverse populations.
Required Credentials, Experiences and Capabilities:
- Bachelor's or Master's degree in Nutrition, Dietetics, or a related field.
- Active Certified Diabetes Educator & Education Specialist (CDCES)
- Registered Dietitian in the state of Florida, Maine, Vermont, Ohio & Delaware or the ability to get licensed within 3 months, is required.
- 3+ years experience in working within an insulin adjustment protocol, or experience in working with insulin adjustments in tandem with a diabetes provider
- 3+ years experience in virtual healthcare delivery and familiarity with telehealth platforms.
- In-depth knowledge about current diabetes treatment and management, particularly in under-resourced settings.
- Strong interpersonal and communication skills, with an ability to connect with patients and co-workers in a virtual setting.
- Commitment to serving under-resourced communities and addressing health disparities.
- Ability to listen actively, communicate with empathy and gather information in a respectful manner.
- Ability to use independent judgment and analyze information from multiple sources.
- Ability to communicate information to medical professionals, clinic employees, coaches, insurance companies and other community organizations over the phone or virtually.
- Language: Fluency in English required, Spanish fluency preferred.
- Communication Skills: Excellent verbal and written communication skills, with the ability to convey complex medical information in a clear and empathetic manner to patients.
- Ability to work effectively, remotely, with people of diverse cultures, ages, and economic backgrounds in a culturally competent and sensitive manner.
- Strong organizational skills to manage and prioritize multiple tasks efficiently.
- Technology Proficiency: Comfortable with virtual communication platforms and proficient in using electronic health record (EHR) systems.
- Adaptable: Ability to adapt to a dynamic virtual work environment and stay updated with the latest advancements in diabetes management.
- Ability to use independent judgment and analyze information from multiple sources.
